What to expect when visiting Lakeside Natural Health Centre

NATUROPATHIC VISIT

If you are visiting our Naturopathic Doctor, your first visit is an initial consultation and is scheduled for 1 1/2 hours, so that we have ample time to understand your health concerns and health goals. We ask that you fill out and return the NEW PATIENT INTAKE FORMS before your first visit. When you come to the office for your first visit, be prepared to talk about yourself, your medical history, your health goals and anything else that you think is important. It is helpful to have copies of your recent bloodwork and other diagnostic tests prior to the first visit. Download the release of Records form and fax it to your medical doctor so that Lakeside may obtain a copy. The second visit involves going over a detailed Health Treatment Plan and often treatment options and suggestions are presented to you at this time. Any subsequent visits are discussed and may involve acupuncture, diet and lifestyle counseling, botanical therapies, or several other treatments. At Lakeside Natural Health Centre we strive to create a simple and lasting health plan that works with your personal goals and priorities.

HOLISTIC NUTRITION VISIT

If you are visiting our Holistic Nutritionist as a new client, you will be asked to commit to two initial appointments. The first visit is an initial consultation and is scheduled for 1 1/2 hours so that we have ample time to understand your health history and nutrition/health goals. Initial recommendations will be provided at that time and a second 1 hour visit will be scheduled. During the second visit detailed recommendations, meal plans, etc. will be provided and discussed.

Please note that prior to the first appointment, we will ask you to complete and return the NEW CLIENT NUTRITION FORMS. Contact the office for a copy of these forms to be faxed, emailed or mailed to you. Please note they are different from the new patient package for a naturopathic visit.

PAYMENT OPTIONS AND CANCELLATION POLICY

Payment Options: We accept all forms of payment – Debit, Visa, Mastercard, cash and personal cheques.

Cancellation Policy: We ask that you provide the clinic with at least 48 hours advance notice if you need to cancel your appointment. Cancellations with less than 48 hours advance notice are subjected to a charge of 100% of the fee for the service scheduled. We appreciate your cooperation with this clinic policy.
